New Patient Registration
Patients living within the practice area are welcome to register, please check the link below to confirm your eligibility from your postcode. If you are eligible please contact the practice to arrange a New Patient Health Check with a member of our nursing team.
You will need to bring along proof of address such as a recent utility bill to your New Patient Health Check appointment.
You will also need to complete a registration form (GMS1) & Pre-registration questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor, which you will need to bring along completed to your New Patient Health Check.
Medical treatment is available from the date of registration. Please contact reception for further information.

Out of Area patients
We will NOT be participating in the "Patient Choice Scheme"
The 'Patient choice scheme' is a government initiative which practices can choose to opt in to. Participating practices can accept patients who live outside of their normal area, but without a commitment to provide home visits.
Wrekenton Medical Group have taken the decision not to participate in the scheme
We feel that if we did, it could lead to an uncontrolled influx of patients which we could not cope with and have the potential to adversely affect the service we provide to our existing patients. In order to continue to provide the best care for our patients, this is a risk that we do not want to take.
