If your are moving house, please be aware that the practice have set NHS practice boundaries which it adheres too.
If your address is outside of our practices NHS boundaries, you will be requested to find a new GP who covers the area within their boundaries where you live.
We fully understand that patients may not always be happy that they have to change their GPs of many years due to a change in home address, but the practice are not accepting patients that do not live within its practice boundaries.
You can check if your address is within our boundaries by viewing our Practice catchment area map and entering your postcode within the relevant section.
We will NOT be participating in the "Patient Choice Scheme" for patients moving outside of the catchment area
The 'Patient choice scheme' is a government initiative which practices can choose to opt in to. Participating practices can accept patients who live outside of their normal area, but without a commitment to provide home visits.
Wrekenton Medical Group have taken the decision not to participate in the scheme
We feel that if we did, it could lead to an uncontrolled influx of patients which we could not cope with and have the potential to adversely affect the service we provide to our existing patients. In order to continue to provide the best care for our patients, this is a risk that we do not want to take.
Please check the Practice Catchment Area if changing your address. If you are moving out of the area, we will continue to provide primary care service to you for approximately 28 days before you are removed from the practice list.
